So I just got my 144hz monitor today, an upgrade from previously using 60hz. The game ran perfectly before on my pc other than it obviously not being 144hz. Whenever I turn on Vsync now, the level goes by my icon in a really choppy way, but my icon still runs at 144fps. It is still mostly playable but it is annoying and causes me to crash every now and then. If I turn off vsync, this problem goes away, but then I can't play at 144hz and I get screen tearing. I've checked my drivers for my graphics card, I've switched around all the combinations with smooth fix and full screen, and the problem does not go away.

Reviving this because I am having the same exact issue with my new 144hz monitor and I have seemingly tried everything.

I don’t use a 144hz moniter, I play with bypass, and I don’t get this problem, but try uninstalling GD and then redownloading it.

Out of curiosity, what graphics card do you have?
