Hm...
Gameplay: 7/10 - It's not bad, but it's pretty inconsistent. There are great gameplays (beginning, 1:58, dual ball), but at other times, the gameplay is either repetitive (2:19, 2:22, 2:33) or just uninteresting (0:16). I would put more obstacles and avoid the copy-paste feeling (even though it may not be the case).
Block Decoration: 4/5 - This part is pretty good. The block decorations are pretty consistent and matches with the music. However, I don't really like that there's only a few block designs; the blocks lack variability. I would mix up the block decorations to give a more decorated feeling.
Background Decoration: 2.5/5 - Mostly, there is a lack of background stuff going on; while there are decorations, the only time I actually see background going on is in the 3-path part and the cloud part (my favorite was the space). Even then, however, the clouds are jumbled up and make the level seem messy. I would add more of these background, even if it was more or less simplistic. Take inspiration from the space path; it had pretty good background.
Effects: 7.5/10 - Not bad; the regular decorations really add to the effects. However, the background really seeps into this part, making it lower than what it could've been. Maybe add a few background flashes here and there, add more to the level, and you should be fine!
Originality: 4/5 - I can't think of a level that comes here. Of course, there is nothing innovative, so -1 for that.
Sync: 4/5 - Sync is, for the most part, good, but at times, the sync is kinda wumbo jumbo (2:18). I would be careful about the duals in particular.
Overall: 29/40 - Decent
I like your unique duals, but they can get jumbled up at times. Also, I dock you points mostly because of the background; it lacks (most of the time). The cloudy parts get a bit messy at times. Clean those up, add more background to the whole level, and be careful with the dual gameplay (dual ball was neat, though).
